SN

SN - Skilled Nursing (Registered Nurses and Licensed Practical Nurses)

Skilled nursing is a term that refers to a patient's need of care or treatment that can only be done by licensed nurses. Examples of skilled nursing needs include complex wound dressings, rehabilitation, tube feedings, or rapidly changing health status. A registered nurse oversees and plans a patients care according to a doctors order. The registered nurse may then direct a licensed practical nurse to continue care, according to a care plan, under the registered nurses supervision.

OT

OT - Occupational Therapy

An occupational therapist helps people who have an illness or disability to do those things that are important and meaningful to them such as eating, dressing, school activities, and work. The occupational therapist helps by making changes in any of the things that may limit an individual's ability to do those tasks, including the environment, the task, or the person's skills needed for the task. Occupational therapists also focus much of their work on identifying and eliminating environmental barriers to independence and participation in daily activities.

MSW

MSW - Masters Social Work

A social worker with a masters degree, usually comes to the home to asses the patients need for additional help to keep the patient as independent as possible. A social worker may help the patient get meals delivered, or help grocery shopping, or may help a patients family with placing a family member in assisted living or nursing care center.

HHA

HHA - Home Health Aide

A home health aide helps patients with activities of daily living, or ADL's. These activities include dressing, showering, shaving, oral care, and personal care. An HHA may also help with household needs such as cleaning, laundry and dishes.

PT

PT - Physical Therapy

Physical therapy is a type of treatment that may be needed when health problems make it hard to move around and do everyday tasks. It helps you move better and may relieve pain. It also helps improve or restore your physical function and your fitness level. The goal of physical therapy is to make daily tasks and activities easier. For example, it may help with walking, going up stairs, or getting in and out of bed. Your physical therapist will examine you and talk to you about your symptoms and your daily activity. He or she will then work with you on a treatment plan. The goals are to help your joints move better and to restore or increase your flexibility, strength, endurance, coordination, and/or balance. Physical therapy almost always includes exercise. It can include stretching, core exercises, weight lifting, and walking. Your physical therapist may teach you an exercise program so you can do it at home.
